Become A Patient

Thank you for your consideration of Granite State Pain Associates. To become a new patient, we require a referral as well as medical records (see below) before scheduling. Please contact your primary care provider or specialist to coordinate your referral.

Last 3-5 Notes from you Primary Care Provider

Last 3-5 Notes from any specialist you have seen in relation to your pain diagnosis

Any imaging reports related to your pain diagnosis from the last 3-5 years

Once your referral is received, a member of our New Patient Referral Team will reach out to you.

Some additional documentation may be needed before scheduling your consultation appointment.

Policy for New Patients

Please be advised that it is not standard practice for us to prescribe pain medications or provide treatment at your initial appointment. If you’re currently prescribed pain medications, you will need to refer back to the provider currently maintaining those medications for refills. If you require procedures/injections to assist in treating your pain, be advised, the provider will need to evaluate you first to determine the type of treatment that will need to be provided before we can perform your procedure/injection. Occasionally, insurance companies require prior authorization for these procedures which may cause some delay in treatment.

Quality pain management usually requires a comprehensive approach. For this reason, additional testing may be required after your initial consultation; e.g., lab work, radiology testing, EMG’s, etc. This appointment is specifically allotted for you and requires more time than a typical office visit.
